Play to Grow OT is IN-NETWORK and able to bill directly with the following insurance plans:

PLEASE NOTE: Being "IN-NETWORK" does not guarantee that you will have no cost - a co-payment or payment in full may still be due at the time-of-service dependent on your insurance plan.
It is the responsibility of the client to check with insurance provider and submit to insurance the invoices provided after each visit to determine the number of sessions that may be covered, and what percentage of cost (if any) will be covered.

All other insurances that are NOT listed above indicates that Play to Grow OT is an out-of-network provider.
PLEASE NOTE: It is the responsibility of the client to check with insurance provider and submit to insurance the invoices provided after each visit to determine the number of sessions that may be covered, and what percentage of cost (if any) will be covered.
A Superbill may be provided to all clients as needed to be submitted to your private insurance for retroactive coverage once invoices have been paid in full.
In addition, Good Faith Estimates will be provided to each client prior to initiating the evaluation process if your insurance will NOT be billed, and you will be paying privately out of pocket.

Comprehensive Occupational Therapy Evaluation: $300

Occupational Therapy Treatment, 1 Hour Session: $250​
